DA Form 31 is used for requesting leave by Department of the Army. Commanders are authorized to produce the DA Form 31 through electronic means.
A member of the Army will type or electronically fill out the leave form. If electronic means are not available, the soldier must legibly hand-print DA Form 31.
Generally, soldiers are entitled to 30 days leave per year and may take leave if the unit mission permits of just a few days or all 30 days after having their DA 31 confirmed by their commander.
Part I, will be used to request personal leave for traveling or vacations and approved by unit commander.
Part II & III, will only used when emergency leave due to family illness or other emergency situations is authorized and approved by authorities shown in the applicable paragraphs in AR 600-8-10. These parts will be disregarded when the form is used for ordinary leave or other authorized absence.
Personal information like name, address, Army rank, social security number, type of leave (ordinary, emergency, permissive TDY), number days leave, departure information, extension and return information is required.
Step 1: Add DA Form 31 PDF file

Step 2: Convert DA Form 31 PDF to Word Format for Editing

Step 3: Edit DA Form 31
