  • Top line: indicate name the individual who is doing the packing;

  • Box 4: indicate the date;
  • Box 5: indicate how many pages this form will run for, and what page of that sequence this particular listing is;
  • Column A: fill in if the total package is contained in more than one shipping box;
  • Column B: describe the particular item in the package (Federal Stock Number (FSN) is preferred entry.);
  • Column C: indicate Unit Of Issue for the item;
  • Last three columns under“QUANTITIES REQUIRED”: optional. Indicate if the item is required for the complete unit being packed to function, or if they are spare parts.
  • Then print and sign DD Form 1750 in the last row.
